While a digital version is available, many caregivers prefer something tangible they can hold, reference, and use daily. To request a kit, simply fill out the form on our website with your name and mailing address. Optional questions help us serve you better, but your privacy is always protected. Hawai‘i kits also include exclusive items like compassion cards, dementia stickers, and a caregiver button. We’re here for you—and honored to walk this road by your side.
